Starting home care should feel simple, supportive, and stress‑free. Our non‑clinical home health care intake process is designed to help us understand your needs and create a personalized care plan that fits your lifestyle, preferences, and goals. Whether you’re seeking care for yourself or a loved one, this first step ensures we provide the right level of support—right from the beginning. 

 

What Is the Intake Process? 

The intake process is how we gather important information to safely and effectively deliver non‑clinical home health care services. During intake, we learn about daily routines, personal care needs, household support, and any special considerations that may impact care. This process allows us to match the client with the most suitable caregiver and create a customized plan of care. 

 

What Information We Collect 

During intake, we may ask for details related to: 


Client Information 

  • Name, address, and contact information 
  • Emergency contacts 
  • Preferred schedule for care 
  • Living arrangements and pets in the home 

 

Daily Living & Personal Care Needs 

  • Assistance needed with bathing, grooming, or dressing 
  • Mobility and transfer support 
  • Toileting or incontinence assistance 
  • Meal preparation and eating support 

 

Lifestyle & Preferences 

  • Daily routines and habits 
  • Food preferences or dietary guidelines 
  • Hobbies, interests, and companionship needs 
  • Cultural, language, or religious preferences 

 

Safety & Home Environment 

  • Mobility limitations or fall risks 
  • Use of assistive devices (walker, wheelchair, shower chair, etc.) 
  • Areas of the home requiring special attention for safety 

 

HealthRelated Information (NonClinical) 

While we do not provide medical care, we may ask general, non‑clinical questions to help ensure caregiver safety and comfort, such as: 

  • Known diagnoses or conditions that affect daily living 
  • Medication routines (for reminders only, not administration) 
  • Recent hospitalizations or changes in condition 

 

What the Intake Process Does Not Include 

Our intake does not replace medical evaluations and does not involve: 

  • Medical diagnoses or treatment decisions 
  • Medication administration 
  • Skilled nursing assessments 


If skilled or medical care is needed, we can help guide you toward appropriate licensed healthcare providers. 

 

After Intake: What Happens Next? 

Once intake is complete: 

  1. We review all information carefully 
  2. A customized, non‑clinical care plan is created 
  3. We match the client with a compatible caregiver 
  4. Care services are scheduled and confirmed 


As needs change, the care plan can be adjusted to ensure ongoing support.
